I authorize ePac Holdings, LLC, its subsidiaries, successors and assigns, clients and agents (“ePac”) to reproduce, publish, broadcast, transmit, display, create derivative works from, and otherwise use, as applicable, my and my Company’s name, image, likeness, voice, biographical material, and other identifying characteristics, or part thereof, as well as my and my Company’s comments, material and testimony, and any other works of authorship provided or otherwise made available to ePac (the “Material”), on a non-exclusive and worldwide basis in any form or medium, now known or hereafter devised, for all purposes, including, without limitation, in connection with advertisements, promotions and marketing of ePac goods and services, either in connection with my own and my Company’s name, or without using my and my Company’s name, or in connection with a fictitious name, with or without written material or testimony, and to make any alterations or additions to Material. I give this authorization and license in exchange for good and valuable consideration, including publicity for my Company and me. I have not relied on any representation made by any ePac employees, agents or contractors regarding the use of any Material. I hereby waive any right that I may have to examine or approve of the use of any Material. I am authorized to grant the rights granted in this Release Form personally and on behalf of my Company. I hereby release ePac and its employees, officers, and directors from any and all liability, claims, demands, lawsuits and damages of any kind (including reasonable attorneys’ fees), arising out of or in connection with the use of Material, including, without limitation, rights of publicity, privacy, or other intellectual property rights. This Release Form will be governed by the laws of the State of Delaware, without reference to conflict of law principles. I consent to the jurisdiction of the state and federal courts located in Wilmington, Delaware and waive any jurisdictional or venue defenses otherwise available.
